• Demographics

• Insurance information

• Our order form, or your electronic order

Fax all documents to 828-322-3160.

2. Timeframe:

We will Precert and schedule all patients and fax referring physicians study results within 2 to 5 business days.

3. Follow up:

Mark on our order form if you would like Dr. Surdulescu to follow up, or your office to follow up after the sleep study.
